Phone Contact:
I check messages periodically during my weekday business hours and generally return calls by the end of the next business day.
For a return call please leave your phone number and the best times to reach you. If I am away, the name and phone number of the psychiatrist covering for me will be on my voice mail greeting.
If it is necessary to cancel a session, you will not be
billed if notice is given at least 24 hours before an
appointment.
My 24 hour cancellation policy allows me to offer the appointment time originally reserved for you to someone else, and helps me run my practice more efficiently.
If you do not cancel your appointment at least 24 hours
in advance, my regular appointment fee will be charged
to you. Insurance plans do not cover fees for
missed appointments or late cancellations.
Prescriptions:
Medication refills are best handled during appointments. If
you find you are running low on a medication between
appointments, call your pharmacy and ask if there is a
refill on file. If
there is not, have the pharmacy fax me a refill request.
My fax number is
(206) 364-0893. Please
allow at least 48 hours for my response to your refill
request outside of appointments.
Insurance, Billing and Payments: Prior to our initial appointment, it will be important for you to contact your insurance company to clarify your mental health benefits. Ask whether you have an annual deductable, a set co-pay, or a defined number of sessions. Your plan may require pre-authorization
I currently am a Preferred Provider for Uniform Medical, Premera, First Choice Health and Regence PPO and PAR plans.
If I am not a
Preferred Provider with your insurance plan, they may
offer benefits for
out of network
psychiatrists. My billing service is
Health Care Administrative Services (HAS) in
Everett. They will verify your insurance benefits,
submit bills for each appointment to your insurance
company and send out monthly billing statements to you. For
insurance and billing questions, please contact HAS
directly at
(425) 258-1880.
I collect annual deductibles at the beginning of the
year and co-pays at appointments. If you do not have
mental health coverage, I ask that you pay my fee at the
time of our appointment. Account balances are due in
full upon receipt of my monthly statement. I do accept
Visa and Master Card payments.
By signing my registration forms you have agreed to be
responsible for the full payment of all fees associated
with the services I provide to you. If you have not paid
your balance (or set up a payment plan), your past due
account may be sent to collections.
If you have any questions about any of these policies,
I'd be happy to discuss them with you.
